you are AGREEING to these terms and conditions and understand these
policies are effective Immediately.**
Registration
Millstone Dance is a 10 month school year from September from June. Students are allowed to register at any point within the year, and tuition payments will begin upon registration.
Payments
I understand payments are due to Millstone Dance by the 1st of each month. Payments can be made in cash, debit/credit card, or check. There will be a $15.00 late fee charge after the 10th of each month and a $25.00 service charge for any check returned from the bank. Millstone Dance does not accept post-dated checks. Tuition remains the same regardless of whether it is a 5 week month or shortened month due to holidays. Tuition is not refunded or credited for missed classes or holidays. No refunds or credits will be given for classes missed. You can set up Bill-Pay through YOUR bank so you never miss a payment!
Tuition
Several payment plans are available and must be selected at time of registration.
Plan 1: Payment in Full (at time of registration) offers a 5% discount.
Plan 2: Two Payment Plan offers a 2.5% discount, payable in two installments – Sept 1st and Feb 1st.
Plan 3: Monthly Installment Plan – tuition is to be paid on the day of or prior to the first class of each month. The first month's payment and registration fee are non-negotiable, non-refundable and is due at registration.

Any student with an overdue balance of more than 60 days will not be allowed into class until payment in full is received or payment arrangements have been made. I will be responsible for these and any and all legal and/or collection fees incurred by Millstone Dance should my account become delinquent.
Refund Policy
No refunds or credits will be given for classes missed. If a make-up class is not available, the student may select an age and ability appropriate level to take as a make-up class.
Schedule Changes
Millstone Dance may alter class schedules and/or teachers for any reason temporarily or permanently.

Weather Condition
Millstone Dance may not follow the Millstone Township schools. You may find our closing schedule for holidays on our website or social media pages.

Parent’s Responsibility to be Aware of Dates and Events
I understand that it is my responsibility to be aware of all important dates and activities. All important dates will either be posted on the Millstone Dance website or in emails . Please make sure you check regularly.

Recital
I understand that I can NOT purchase recital tickets for any other dancer but my own, and in order to receive recital tickets my balance MUST be paid in full by ticket day. If my balance is not paid in full by ticket day, I understand I’m still allowed to pick my tickets, but they will be held by the director till all payments are paid. If ALL payments fail to be in by dress rehearsal, I understand that I will not be allowed to attend the show and/or have my dancer dance in show. Costumes are ordered well in advance of recital. Costume payments are due no later than December 10th. No refunds or exchanges are offered on costume orders. If student is unable to perform in the recital, teacher and the office must be notified in writing otherwise a costume will be ordered. I understand in order to receive ANY of my costumes ALL balances MUST be paid in full.
Accident or Injury
I agree to provide the necessary medical insurance for my child/children and I will not hold Millstone Dance or employees liable in the event of any accident or injury. This is a physical activity. By participating, there is an assumed risk of injury. If an injury does occur, I understand Millstone Dance is not financially liable. If I cannot be reached in an emergency situation, I give my permission to the Millstone Dance staff to render or act on my behalf to obtain emergency medical treatment for this student for any illness or injury that may occur while attending the studio. Millstone Dance is not responsible for any injuries that occur to any siblings. All children that are not in the classroom with a teacher MUST be watched all times. As the legal parent or guardian, I release and hold harmless Millstone Dance, its owners, agents, servants and assigns from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control of Millstone Dance, its owners and operators or in route to or from any of said premises.

Photo Release
All recordings, including but not limited to video, audio, still pictures, motion pictures of any event, including but not limited to recitals, rehearsals, dance acts, classes, demonstrations, and/or competitions remain the property of Millstone Dance with all rights of ownership thereby retained. Millstone Dance is hereby granted permission to take photographs or videos of the students to use in brochures, web sites, posters, advertisements, and other promotional materials the school creates. I understand the student's name may be used in conjunction with a picture for studio newsletters / programs only.
Millstone Dance reserves the right to drop any registrant from our registrar for:
a. Excessive lateness and/or absence;
b. Disruptive behavior;
c. Non-payment of tuition and/or other charges

Social Media
Do not publish any content that would be damaging to Millstone Dance, its employees, teachers, and students.

Dance Class Etiquette
We understand due to certain circumstances dancers will be late. Please call the studio and let the front desk know how tardy your dancer will be so they can inform the teacher.
I acknowledge that the original of the agreement and enrollment form will be held on file in the office of Millstone Dance and that I may obtain a photocopy of it upon request.
